if you contact the records dept. of the hospital where you were dx’d with DM
and give them a time frame, they can probably send you dated BG reports,
copies of invoices, etc. to prove this. Send copies to:

And you will receive their bronze 50-yr medal. They in turn will alert Eli
Lilly Corp. who will send a sterling silver medal engraved with your name on
it with a s.s. chain. Out of all the DMers in the world, I’m in the 1,455
medals they have issued since the program began in 1974.
Also, anyone proving 25 years of IDDM, send to the records dept. and you will
be issed a certificate.
I held my 50-year BASH (Bawling, Amazement, Sincerity, Humor) 4/11/00 (actual
