I enjoyed all the responses to the "sneaking food" dilemma — every
thought I had while reading the original request was quickly posted by
someone else, along with some brilliant ideas I never would’ve come up
with.
Bart, congratulations on your startup…I hope it’s still going well!
Arlene, how did your return trip go (if in fact you flew home)?
Jan, I grinned at your description of meeting a fellow pumper!
Today is my two-month pumping anniversary, and I’m amazed both at how
fully it’s integrated into my life AND at how many things I still don’t
know. Here’s one:
My CDE’s notes say that ketones automatically require a site change,
which I’d forgotten — the last few times I’ve had ketones I bolused,
drank a ton of water and saw them vanish on the next test 40 minutes
later. So is there some reason for this rule that I’m just not seeing,
or has anybody else been skipping the ketone-site-change and doing okay
with that?

Laurie, the reasoning goes something like this. Ketones are usually caused by
high bg’s. Therefore, you must not be getting enough insulin, due to a
blockage, etc., so a site change is indicated.
Sometimes, though, if you bolus a large amount the blockage can be "blown
out", depending on how severe it is and what’s causing it. It’s like
everything else, just use a little common sense and your own experience.
Everything is NOT carved in stone!

I do much the same as you, if I feel it is not a site problem. I do check
in one hour and if the BGs are not going down and the ketones I then change.

if your bs returns to normal or drops significantly then your site isn’t bad,
but i guess when using a site that has a cannula in it it can be kinked for
some time and no insulin is being injected and therefore you run the risk of
dka.
however you can get ketones other ways not just from lack of insulin, you
could be losing weight, or exercised really hard or sick.
you sort of have to use your own judgement when to change sites, some people
seem to change them several times in a day, and we only change once in five
days and her numbers are still good, and her site is without redness or
soreness and is completely healed the next day. but she uses rapids. we use
five days as it gets her threw till she really needs to fill more insulin
syringe as she hates changing her site when the tape is still stuck really
good.
